Man crushed to death by falling glass case

A Leeds-based freight company has been given a major fine after a worker was crushed to death at a container base in Stourton.
The man was with two cargo handlers and a supervisor while glass cases were being unloaded from an open-top freight container at the site on April 6th 2006.
Seeing that the final case, which weighed two tonnes, was in danger of falling, the admin manager attempted to intervene but was crushed by it, suffering fatal injuries.
Leeds Crown Court heard that the company involved in the incident, Roadways Container Logistics, had failed to give any of the workers appropriate lifting operation training.
In addition, an investigation by the Health and Safety Executive revealed that no risk assessment of formal planning had been carried out and that the case was not properly secured.
The firm pleaded guilty to breaching Section 2(1) of the Health and Safety at Work etc Act 1974 and was fined £250,000 and ordered to pay £100,000 in costs.
HSE inspectors urge companies to implement appropriate measures such as safety signs to reduce the chance of accidents occurring in the workplace.
